Form Pilates MT — Studio Policies & Terms
Late Cancel & No-Show Policy
To ensure everyone has a fair chance to book a spot, we have a clear cancellation policy:
Late Cancellations (within 8 hours of class): You’ll be charged a $15 late cancellation fee.
No-Shows (if you’re not present at the start of class): You’ll lose the class credit and be charged a $20 no-show fee.
We understand that emergencies happen; however, we strictly enforce this policy to maintain fairness and sustainability.
All cancellation fees are non-refundable and non-negotiable. By booking a class, you authorize Form Pilates MT to charge the payment method on file in accordance with these terms. We reserve the right to change this policy as needed.
Waitlist Policy
If you’re added to a class from the waitlist, you’ll receive an email. By accepting the spot, you’re committing to attend class.
Please note:
Once you accept the spot, it becomes your responsibility to attend or cancel in time.
If you can no longer attend, please remove yourself at least 8 hours in advance to avoid fees.
Late cancel and no-show policies apply once you are enrolled in the class, even if you were added from the waitlist.
Class Packs & Membership Expiration
Expiration dates for all class packs and memberships are clearly outlined at the time of purchase. It is your responsibility to review these details.
All class credits are non-refundable, non-transferable, and may not be extended beyond the stated expiration period.
Memberships require a 3-month minimum commitment — no exceptions, unless otherwise agreed to in writing under extenuating circumstances (e.g., injury, relocation).
A 3% credit card processing fee applies to all purchases.
All sales are final. Form Pilates MT reserves the right to deny refund or extension requests that fall outside of these stated terms.

Grip Socks Required
For safety, hygiene, and equipment preservation, grip socks are required for all classes. If you forget yours, a limited number may be available for purchase at the studio. Failure to wear grip socks may result in being asked to sit out of class for your own safety — no refunds or credits will be issued.
Studio Conduct & Etiquette
We kindly ask that you:
Arrive early to settle in. If you’re more than 5 minutes late, we may not be able to admit you to class, both for your safety and to minimize disruption.
Treat instructors, staff, and fellow clients with kindness and respect at all times — any disruptive, aggressive, or inappropriate behavior may result in suspension or termination of access to the studio without refund.
Notify your instructor of any injuries, limitations, or special considerations before class begins. Your safety is our priority, but ultimately, you are responsible for honoring your body.
Additional Terms & Conditions
All class bookings require a signed liability waiver on file. Clients must re-sign annually or as requested.
We reserve the right to cancel, reschedule, or modify class formats at any time due to illness, inclement weather, equipment issues, or emergency circumstances. If a class is canceled by the studio, your credit will be returned to your account.
Form Pilates MT is not responsible for lost, stolen, or damaged personal property on studio premises.
We reserve the right to refuse service to anyone whose behavior compromises the safety, comfort, or wellbeing of others.
By booking a class, you acknowledge that you have read and understood these terms, and agree to follow all studio policies in effect.
